How to Register & Pay by Mail
- Download the Registration Form
- Complete a Camp Registration form for each child
- Complete the Medical & Emergency Contact Information form for each child
- A copy of each child’s recent physical including complete immunization records must be submitted 2 weeks prior to the program start date
- Make your check payable to FHHF
- Mail to: Friends of Holly Hill Farm, 236 Jerusalem Road, Cohasset MA 02025
- Or hand deliver to the Farm Office during business hours
- Payment by credit card may be made by calling the Farm Office to set up an appointment.
Upon receipt of the registration form including medical and emergency contact information, current physical and immunization records, and payment you will receive an email confirming the registration process has been completed.
